JESUS Film Mission Trips - APPLICATION INFO _

Team members are responsible for the costs of their own trips. You may pay for it yourself, or trust God to raise the needed funds (approximately $1,500 – $4,000). For some this is intimidating, but it doesn't have to be. It will be a tremendous faith-building exercise and is done all the time by ordinary believers. We will provide you with a guide booklet about support-raising, which will help walk you through the process. Click here to learn more about support raising.

(You will need to have Adobe Acrobat Reader to read and print several of the forms below.)

Step 1:	Fill out and submit the online Application. If you prefer, you can download the Application instead and mail it in to the address below with the other forms.
Step 2:	Download and print TWO copies of the Reference Form and give them to two people who know you well (such as pastors or spiritual mentors) to complete and send in.
Step 3:	Download, print out, and sign the Liability Release Form and have it witness-signed by another person.
Step 4:	Download, print out, and complete the Medical Authorization and Release Form.
Step 5:	Make a photocopy of the first page of your passport.
Step 6:	Make out a check or money order for $100, payable to Campus Crusade for Christ, for your non-refundable application fee.
Step 7:	Enclose items #3–6 (and your application, if you did not fill it out online) in an envelope and mail them to us at:

Please note that your application process is not complete until we receive all of the necessary items.
